1. Log-on to: https://dennys.ushealthcenter.com/Activate
2. Select Activate by Demographics or Email.
3. Enter your personal information to create your account. You will receive your username.
REMEMBER THIS FOR FUTURE LOGINS!
4. Select Login and enter your password
5. Congratulations! Your PHD account is now activated and you are logged in.

Once you have enrolled, you will receive a Welcome Packet delivered to your place of employment with your personal ID card, an overview of your plan, and information on how to access your benefits. Please keep this packet, as it contains details you need to register and access your benefits.
To get started, download the WorkChoice App by scanning the QR code to the right or visit www.workchoicemembers.com and create your account. Note that your benefits information will not be available on the app until after your plan's effective date.
